As a Member Navigator, you will play a vital role in engaging, educating, and supporting Unifi members throughout their healthcare journey.  This role is a combination of health benefits and healthcare services navigation with an emphasis on service delivery.  Members will view you as their personal guide as you support them through what is often a challenging time.  As a part of this team, you should thrive on solving problems and be prepared to step in at a moment's notice with empathy, expertise, and passion to advocate on behalf of our members.

What You’ll Do:

  •  Interact with members in group and individual settings to educate them on the unique aspects of their UnifiHealth plan as well as the value of Advanced Primary Care and chronic condition management
  • Help facilitate member education and enrollment with advanced primary care services and work as a conduit between providers and community resources to ensure a coordinated, high-quality experience
  • Help facilitate member education, enrollment and utilization of Advanced Primary Care services when applicable and maintain relationships with Advanced Primary Care partners to develop an integrated care coordination and referral management process
  • Assist members in navigating the healthcare system by providing information about available healthcare services and helping coordinate visits between specialists and other available resources in the community
  • Work closely with leadership and data teams to develop population risk management strategies
  • Work closely with a growing cross functional team to analyze reports and develop member engagement campaigns
  • Support sales, marketing, client success and product by representing the UnifiHealth member advocacy program in internal and external meetings
 

As the Member Navigator, you should have:

  • 3+ years’ experience in working in healthcare setting or case management role 
  • An understanding of clinical operations and be able to explain the scope of services available to members in their primary care facility
  • Knowledge of the healthcare industry and available patient resources
  • Good written and oral communication skills
  • Comfort presenting in front of an audience
  • A service mindset and passion for optimizing the member experience
