The Manager of Case Management is an important part of the interdisciplinary care team. They are responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting. Function may also be responsible for providing health education, coaching and treatment support for members.
Responsibilities
Sets team direction, resolves problems and provides guidance to members of own team.
May oversee work activities of other supervisors.
Adapts departmental plans and priorities to address business and operational challenges.
Identifies and resolves operational problems using defined processes, expertise and judgment
Influences or provides input to forecasting and planning activities.
Reviews work performed by others and provides recommendations for improvement.
Forecasts and plans resource requirements, tracks client and program reports ensuring performance targets are met.
Accountable for integration of the assessment for the plan
Perform initial review of consumer health records and current health status (e.g., health risk scores) to identify health risk and determine next steps
Receive information from team about specific symptoms and questions
Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health literacy
Evaluate member data throughout the assessment process to identify inconsistencies and adherence to Evidence Based Guidelines
Identify and prioritize gaps to develop plan of care to empower members to meet identified goals
Determine member’s willingness to engage in plan of care and readiness for change
Document assessment information and enter data in appropriate system

Qualifications
Required Qualifications:
Current, unrestricted RN license
Ability to get licensed in other states as needed
3+ years of RN experience
Knowledge of existing KRS systems, business, processes
Renal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your job
Computer proficiency, to include strong data entry, utilizing MS Office (Word, Excel, PowerPoint and Outlook), and the ability to type and talk at the same time while navigating a Windows environment
Reside in a location that can receive a high speed internet connection or can leverage existing high-speed internet service
Strong written and verbal communication skills
Attention to detail, organizational skills and multitasking abilities will be keys to success
Willingness and ability to obtain additional state specific RN Licensure(s), if necessary
Preferred Qualifications:
- BSN or higher level of education
- Certified Case Manager (CCM)
- Diabetic Educator experience
- Field-based experience going into homes
- Telephonic case management experience
- Experience working within a managed care environment
