About the Role:

At OncoveryCare, we deeply understand the impact of cancer on a patient’s physical and mental health, and we are dedicated to supporting our patients with trauma-informed and evidence-based care. As our Survivorship RN Care Navigator, you will play a crucial role in connecting patients with the personalized resources they need to thrive. Collaborating with our multidisciplinary team—including survivorship MDs, NPs, LCSWs, and Peer Guides—you will guide cancer survivors and their families as they transition from active treatment to survivorship. Your will span triaging survivor communication and symptom management, escalating issues to respective medical teams, identifying resources, providing educational support, and serving as a vital link to our community referral partners. You will also have an opportunity to help shape the future landscape of survivorship care through working closely with executive leadership on program delivery and patient experience.

What You’ll Do:

  • Triage Clinical Needs: As a first point of contact on our medical teams, you will have direct interactions with patients regarding their signs, symptoms and questions
  • Care Navigation: Collaborate with the care team to ensure seamless access to medical, psychological, and social services.
  • Care Plan Communication: Empower patients’ understanding and engagement in their care plan, bridging communication between patients and their clinical teams.
  • Educational Guidance: Provide survivors and families with evidence-based educational materials from OncoveryCare
  • Foster Community Collaboration: Build connections with local community partners and develop a strong network of referrals for survivors across all domains
  • Patient Advocacy: Serve as a steadfast advocate for our survivors, identifying their goals and supporting their engagement with OncoveryCare and community services

How You’ll Thrive:

  • Within the first 90 days, you will onboard with our care team and familiarize yourself with OncoveryCare’s philosophy and approach to survivorship. You will learn about our clinical programs, our triage protocols, community care partners, begin to engage with patients and assist them with care coordination needs
  • Within the first year, you will gain mastery of the healthcare ecosystem of cancer survivorship, build strong relationships with OncoveryCare patients and community partners, and provide critical feedback and insight to OncoveryCare’s leadership team on program delivery
  • Long-term growth opportunity: You will have the chance to rapidly progress within a dynamic startup and play a pivotal role in the design and delivery of survivorship care programs for cancer survivors.

What You Will Need:

  • 5+ years of experience in a healthcare setting with specific leadership expertise in care navigation, exposure to referral processes, customer service, and basic clinical triage in oncology or survivorship care
  • Passion for survivorship and track record of professional work addressing the needs of this population
  • Bachelor's degree in a healthcare-related field (e.g., nursing)
  • A Registered Nurse (RN) license and willingness to potentially obtain additional state licenses
  • Ability to educate patients on complex medical information in an understandable manner, adapting to individual patient needs
  • Excellent communication, empathy, and active listening skills
  • Ability to drive work forward independently and collaborate effectively with diverse stakeholders
  • Strong consideration will be given to candidates with lived experience of illness or caregiving
  • Full-time availability for a 40-hour workweek, Monday to Friday, 9 am to 5 pm (flexible)
