As a fully remote clinical nurse at knownwell, you will play a crucial role in our outpatient primary care and metabolic healthcare practice, providing high-quality, patient-centered care coordination and support. This position focuses on managing inbound patient communications, triaging patient needs, and facilitating appropriate care for both new and established patients. The clinical nurse is a key point of contact for many patients, supporting an effective, organized, and compassionate care environment in collaboration with the interdisciplinary care team and office staff.

Schedule:Full Time Monday-Friday with flexible scheduling (mix of patient contact hours plus administrative time).

Responsibilities
- Monitor and triage inbound patient messages via electronic health records (EHR) system, phone calls, and other communication channels.
- Conduct preliminary screenings of new patient inquiries and self-scheduled appointment bookings to evaluate primary complaints, metabolic concerns, and other relevant health information to determine scheduling needs and ensure the correct appointment type and duration
- Coordinate new patient intake processes and ensure all pre-visit information, including patient histories and initial lab requests, are documented and accessible for providers.
- Work with medical assistants, front desk, and other staff to ensure patients receive timely follow-up for lab results, referrals, medication management, and treatment plans.
- As requested, communicate/educate care plans and follow-up instructions to patients in an accessible and supportive manner, addressing any questions or concerns.
- Deliver patient-centered education on disease prevention, chronic condition management, medication adherence, and lifestyle modifications relevant to primary and metabolic health.
- Educate patients on routine screenings, health maintenance, and preventive care appropriate for their age, gender, and health status.
- Offer condition specific education/guidance for patients as needed to support understanding and engagement in their plan of care
- Support patients in understanding and engaging in recommended lifestyle, dietary, and physical activity guidelines to improve health outcomes.
- Ensure timely and precise documentation of patient interactions, assessments, care plans, and follow-up activities in the EHR system.
- Maintain clear and organized notes, ensuring all communications, interventions, and recommendations are thoroughly recorded and accessible to the care team.
- Comply with clinical policies, regulations, and standards of care, maintaining confidentiality and HIPAA requirements.
- Participate in quality assurance activities, audits, and reviews to uphold and enhance clinic standards.
- Stay informed on best practices in triage nursing, metabolic health, and primary care. Participate in ongoing training, professional development, and team meetings to continually enhance care delivery.
- Provide feedback to clinical management on operational improvements and contribute to process enhancements that promote a patient-focused, efficient clinic environment.
Stay current with licensure and appropriate re-certifications
Requirements
- Active unrestricted Registered Nurse (RN) license in a compact licensure state or licensure in MA with willingness to obtain additional state licenses as requested by knownwell
- Bachelor of Science in Nursing (BSN) preferred
- At least 5 years of clinical experience overall, including pre-nursing
- At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
- You have excellent communication and interpersonal skills.

- Must have access to high-speed internet and phone lines to support a fully remote practice.
