This is a full-time on-site role located in Minneapolis, MN for a Community Paramedic. The Community Paramedic will be responsible for providing in-home medical care, including routine check-ups, urgent care services, and health education. Daily tasks will include biometric monitoring, digital surveillance, telehealth engagement, and coordination with virtual care teams. The Community Paramedic will also engage with patients' medical providers to ensure seamless care transitions and optimal health outcomes.

• Clinical skills: Advanced life support, emergency care, patient assessment, and medication administration
• Technical skills: Proficiency in using biometric monitoring devices, digital surveillance tools, and telehealth platforms
• Interpersonal skills: Excellent communication, patient engagement, and teamwork abilities
• Administrative skills: Documentation, coordination with medical providers, and care planning
• Current paramedic certification and valid driver's license
• Bachelor’s degree in a related field or equivalent experience is a plus
• Previous experience in community health or home-based care is an advantage
