As a Remote Triage RN you will play a crucial role in providing remote triage, responding to patient vital signs, and providing support to patients receiving hospital-level care in the comfort of their homes during the evening and night shifts. You will be responsible for assessing patients' needs, providing guidance, and coordinating appropriate care interventions. This remote role ensures that patients receive high-quality healthcare services, maintains effective communication with the care team, and supports patients and their families during the overnight hours (11pm-7:30am).

Duties & Responsibilities:

  • Remote Triage:
    • Provide telephone or video-based triage support for inbound triage calls from patients or caregivers.
    • Assess patients' symptoms and needs to determine the appropriate nursing interventions.
    • Collaborate with the care team to make informed decisions about patient care.
  • Remote Vital Sign Monitoring:
    • Respond to vital sign alerts in alignment with Inbound Health’s protocols and expectations
    • Assess the patient’s symptoms and needs and communicate to appropriate team members
  • Virtual Medication Administration:
    • Provide video-based medication administration by crosschecking prescriptions in the home with electronic health records.
    • Provide support to patients and caregivers to ensure proper technique and adherence to self-administered medications.
    • Educate patients and caregivers on medication use, side effects and answer medication questions.
    • Coordinate with physicians, pharmacists, and care teams to ensure continuity of care.
  • Patient Support:
    • Educate patients and their families on managing their conditions and following care plans.
    • Offer emotional support and address patient concerns and questions.
    • Monitor patients' conditions remotely and respond to changes promptly.
  • Coordination:
    • Care plans for incoming patients
    • Communicate the care plan effectively with the appropriate healthcare providers, physicians, and other care team members.
    • Ensure seamless care transitions and coordinate necessary interventions.
    • Document patient information accurately in respective electronic health records (EHR).
    • Case finding to aid in expanding patient admissions.
  • Emergency Response:
    • Provide guidance to patients in case of emergencies and assist in coordinating emergency services when necessary.
    • Collaborate with local emergency services as needed.
  • Quality Assurance:
    • Adhere to evidence-based clinical guidelines and best practices in patient care.
    • Participate in quality improvement initiatives and contribute to process enhancements.
  • Technology Utilization:
    • Utilize telehealth technology, EHR systems, and communication tools effectively.
    • Stay updated on new technologies and best practices in remote patient care monitoring.

Education & Experience Requirements:

  • Completion of an Associate’s, Bachelor’s, or Master’s degree program in Nursing. BSN strongly preferred.
  • Two (2) years of RN experience in a healthcare setting. Preferred candidates would have at least 2 years of home care, acute inpatient care, or emergency department experience.
  • Previous experience in telehealth, remote triage, or case management is desirable
  • Excellent communication skills, both verbal and written.
  • Experience with technology including EMR systems and Windows based computer skills.
  • Current Registered Nurse (RN) state licensure required in MN, WI, and / or NJ.
  • BLS Certification
