Job Responsibilities:
- Clinical Care (Hospital at Home RN Duties)
- Deliver acute-level care in patients’ homes, including assessments, interventions, and monitoring.
- Perform nursing procedures such as IV therapy, wound care, and medication administration.
- Collaborate with virtual care teams and interdisciplinary providers for integrated care plans.
- Evaluate social determinants of health and incorporate them into personalized care planning.
- Manage biometric monitoring devices and respond appropriately to data and alerts.
- Educate patients and families on conditions, treatments, and discharge planning.

Liaison & Coordination Responsibilities
- Screen and assess patient eligibility for Hospital-at-Home services.
- Coordinate smooth transitions from the acute setting to the home environment.
- Attend discharge huddles and communicate recommendations to inpatient teams.
- Cover for the Clinical Liaison as needed, including on-site presence in hospitals and coordination with hospital discharge planning teams.

Logistics, and Program Support
- Assist with logistics and supply chain coordination, including tracking, ordering, and deployment of clinical supplies and technology.
- Help develop and refine workflows, tools, and standard operating procedures.
- Support onboarding and orientation for new RNs or liaisons.
- Participate in root cause analysis, safety event reviews, and quality improvement initiatives.
- Promote and model excellence in documentation, communication, and compliance.
- Represent the NJ program in internal meetings and external partner engagements.
Work Schedule and Location:
- 1.0 FTE position
- Occasional weekend and holiday rotation based on patient needs and coverage
- Must be located in the Paramus, NJ area to see patients on site and in the home
Our ideal candidate is “patient-centric” and wants to help us shape the future of healthcare. They will bring:
- Registered Nurse (RN) – active state licensure (NJ) or ability to obtain by start date
- Associate’s/Bachelor’s Degree in Nursing from an accredited nurse program
- 2+ years of nursing experience
- Previous experience in acute/ICU/ER/home health setting highly preferred
- CPR/BLS certification
- Valid driver’s license
- Excellent interpersonal, (verbal/written) communication, and organizational skills
- Knowledge of medical terminology, procedures, and treatments
- High clinical proficiency in nursing
- Proficient in technology including EMR systems and Windows based computer skills
- Knowledge of state and federal home health regulations
- Ability to exercise independent judgment and initiative
